If the requirement is to fill the different set of fields for the different case types you can create record types and for each record type you can assign a different page layout by including a different set of fields.
After creating the multiple record types, if you click on New case button you will see an intermediate screen where you can choose the type of record type based on that it will show the corresponding page layout. If you are displaying same picklist field in multiple record types and requirement is to show different picklist values based on the record type then you can configure in record type detail page.

Second question - in order to map web to form leads to the appropriate Record Type, does "Keep existing record type" in Lead Settings have to be selected?
Third question - I have an existing web to lead form that is feeding into a separate Record Type. In the original HTML code, record type which is "Default record type" was not specified since I only had one record type at that time. Will I have to redo the web to lead form, specifying the record type and replace the old code or will leads continue to feed normally?

First question - only recrodtypeID Second question - only applicable when manually creating leads NOT for web to lead Third question - "The assignment rules determine the record type when a lead is created manually or imported without a specified record type.
When no assignment rules apply, the record type of the lead owner is used" -- So, the record type of lead will depend on who it is assigned to and thus will take default record type of that user.
